We accept most major insurance, Worker’s Compensation, No - Fault Auto insurance and Medicare. Please call if there are any questions regarding your coverage.
Please bring the following to your scheduled appointment:
- Reports of MRI, CT Scan, X-rays, EMG, nerve conduction studies
- Pertinent Medical Records from other treating physicians
- List of all Medications that you are currently taking
- Your insurance card / information
- Names and addresses of referring and primary doctors
- Completed patient information forms (if possible)
